Transición hacia un modelo de formación por competencias en perfusión cardiovascular, revisión narrativa estructurada

Resumen

La perfusión cardiovascular en el contexto de la circulación extracorpórea (CEC) y la oxigenación por membrana extracorpórea (ECMO), se configura como un proceso sociotécnico altamente complejo que trasciende la ejecución de habilidades técnicas aisladas. Por ello el objetivo del presente artículo fue realizar una revisión narrativa estructurada de literatura científica sobre los factores que influyen en el proceso de ensamblaje del circuito extracorpóreo, para su valoración, mediante la articulación de las competencias profesionales técnicas y no técnicas y su impacto en la seguridad clínica del paciente. Se llevó a cabo una revisión narrativa estructurada de la literatura científica publicada desde el 1 de enero del 2021 hasta el 1 de junio del 2026 en inglés y español, centrada en la seguridad del paciente, la psicología del error, las guías de prácticas internacionales, así como los modelos de formación por competencias aplicados al ensamblaje y conexión del circuito extracorpóreo. Los hallazgos refuerzan que el factor humano es un precursor de eventos adversos, corroborando el hecho de que la habilidad técnica por sí sola, aunque necesaria, no es suficiente si no se pone en práctica consistentemente con habilidades no técnicas (NTS) como son la conciencia situacional, la toma de decisiones o la autorregulación conductual. En este sentido, la investigación corrobora cómo la vigilancia cognitiva mantenida y la regulación del comportamiento profesional operan como una barrera de las fallas latentes, permitiendo interrumpir el automatismo de las desviaciones y eventuales fallas en las mismas. El aporte más importante consiste en interpelar la incorporación de modelos formativos por competencias que incorporen la simulación de alta fidelidad y herramientas para auditoría conductual, como el instrumento PINTS, que cambiaría la identidad del perfusionista de operario técnico a actor clínico estratégico, gestor de riesgos, fortaleciendo la existencia de una cultura profesional por la seguridad del paciente.
